Building on family’s strengths, Key Workers offer service in three different levels of support. Some examples of the service provided in each level are:

Level 1

• An information package about services available in Delta
• Someone to attend meetings or to facilitate the filling out of forms
• Assistance with referral to diagnosis

Level 2

• Families will receive a Family Action Plan outlining their priorities for service, which agencies within the community will do what and the time frames for completion as well as an in-depth child profile
• Resources and materials to assist with implementing their Family Action Plan
• Care team meetings to proactively assist them with their priorities

Level 3 (Typically for families with children who have a diagnosis)

• Families may access behavioral support if their child is presenting with on-going challenging behaviors. Behavioural support is an intense service, which requires long-term time commitments from the family. It involves: identifying priorities, assessment, and plan development, plan implementation and monitoring and modifications
• Families will meet on a weekly or bi-weekly basis with the Behavioural Consultant in the initial stages of identification and assessment. Once a Behavioural Support Plan is implemented, and families have acquired new strategies, meetings will be less frequent.
• As families become more comfortable with their knowledge and skills, they may receive monitoring services.

Entry criteria:  DCX
Anyone may refer a child for services as long as the legal guardian has given verbal or written permission. Children must:
• be residents of British Columbia, and living in Delta (catchment area)
• be between birth to 19^th birthday
• a diagnosis is not required however, the child must have significant delays in at least one area of development, have mental health concerns or significant behavioural concerns

Transition criteria:
Sometimes there is a need to transition to other levels of services or a transition within a program/service.
Transition to another program/service may be needed due to following reasons:
• a change in need, physical and/or non-physical
• a change in environment
• a change in Reach personnel

Exit criteria:
A child will be discharged:
• on their19^th  birthday
• They reached their goals – this will take place in conversations and agreement with the legal guardian

• They exceed one year of service
  • They move out of Reach’s catchment area 

All families in Delta Connex have access to our parent/peer support groups and workshops. Workshops are offered for parents, professionals, and others in the community on such topics as Positive Parenting, Challenging Behaviours, Understanding Visual Supports, Sexuality, and FASD Prevention.
